JourneyPure is seeking a Full-Time Case Manager for our JPATR Murfreesboro, TN site.

Job Title: Case Manager

Job Summary:

This position requires a thorough understanding of facility’s treatment program and philosophy, admission process and criteria, support and administrative services, medical knowledge of various aspects of alcoholism, drug abuse and dual diagnosis. This is a moderately complex position requiring the individual to serve as a primary link between counselors, clinical technicians, clients, family and other agencies regarding client care.

Job Functions:

Work is sedentary or ambulant. Must read, write, and speak English fluently. Occasional physical exertion (lifting 30 or more pounds, walking, standing, etc.) to support client weight in case of emergency situation or client disability requiring assistance in walking, standing, etc. Must be able to see, stoop, sit, stand, bend and reach. Must be able to relate to and work with ill, disabled, emotionally upset, elderly, hostile people as needed. Must be able to communicate verbally and in writing with the medical, nursing, clinical and other staff. Must be emotionally stable and exhibit ability to display coping skills. Risk of exposure to infections and contagious/blood borne diseases. Exposed to varying and unpredictable situations (i.e., client behavior). Subject to work schedule and shift change.

Qualifications:

  • Education—Bachelor’s degree or above with a major in chemical dependency, psychology or social work, or counseling.
  • Experience—Experience in substance abuse or mental health preferred.
  • Certifications—CPR
  • Licenses—None required
  • Special Requirements—Knowledge of chemical dependency and dual diagnosis treatment and rehabilitation requirements.
  • Age/Gender Specifics—Ability to work with male and female, adult (18-over) substance abuse/dual diagnosed population.

Position Classification: Non-exempt

Essential Duties and Principle Responsibilities:

  • Establish, maintain, and manage communication with key referral sources.
  • Maintain patient records through EMR.
  • Use of recovery-oriented, person-first language in all interactions, with a strong understanding and ability to demonstrate excellence in customer service.
  • Assists residents in obtaining and completing FMLA and short-term disability paperwork.
  • Request medical records and coordinate care with other facilities or providers.
  • Assist in legal needs on behalf of the resident.
  • Ability to manage caseload.
  • Maintain and protect confidentiality and security of all organizational information gained in the course of performing job responsibilities including, but not limited to, financial and client information.
  • Performs other duties as assigned.
