Clinical Educator ~ Home Health and Hospice

The primary responsibility of this position is to accomplish positive outcomes in patient satisfaction and staff education. The Educator interacts directly with employees, patients, and supports Home Care and Hospice programs. Initiatives staffing educational needs and focus on ensuring a consistent and exceptional patient experience. The Educator analyzes patient experience survey results, monitors quality improvement processes and identifies, prioritizes, plans, and oversees improvement efforts resulting in a culture of clinical and patient excellence. The Educator develops and ensures effective and consistent implementation of training and education strategies for the Home Care and Hospice departments. Provides coaching to clinical and support staff through education activities and personal coaching, and training to ensure competency of staff at all levels of the organization.

Essential Functions:
  • Develop and implement structured orientation for home care and hospice clinical staff: RN, LPN and HCA
  • Serve as nursing mentor with staff while learning admission, recent, discharge and resumption of care visits processes
  • Perform direct supervisory visits in the field with clinical staff
  • Review and revise home care policies as directed
  • Assist with the development of structured orientation for other disciplines
  • Mentor clinical staff on home care processes (i.e. medication, reconciliation, recertification, POC development)
  • Develop and present pertinent education to meet agency goals
  • Demonstrate and encourage team behavior to create exceptional patient /guest experiences.
  • Develop and implement skills validation process including oversight and utilization
  • Quality monitoring for educational needs
  • Identify and implements best practice experience improvement strategies and support a standard of care that improves the Patient Experience, increases employee engagement, and favorably impacts the fiscal performance.
  • Uphold and promote patient safety and quality.
Requirements:
  • Active AL RN license required. Associate’s Degree required; Bachelor’s degree preferred
  • 1 year of experience in quality improvement projects
  • 3 years experience in a clinical setting with direct patient care required.
  • 3 years experience in Home Care and Hospice education program preferred.
  • Oasis educated preferred
