Behavioral Health Home Care Coordinator

The Opportunity Alliance is seeking a Care Coordinator to join our Behavioral Health Home team. The Care Coordinator is a direct service staff position for the Behavioral Health Home (BHH). This position contributes to program outcomes through the provision of care coordination for BHH members supporting a recovery oriented, integrated health service that strives to improve health outcomes, cost effectiveness, and enhanced consumer engagement. Work is conducted at members’ homes, at providers’ offices, at TOA locations and in other community settings. Care Coordinators will collect data, participate in program initiatives, attend team meetings, supervision and work groups, coordinate service teams for each member, use information to improve health outcomes and complete documentation in a timely fashion.

Qualifications:
  • BSW or MSW preferred (Bachelor's and/or Master's Degrees in psychology, social services, counseling, rehabilitation, and nursing maybe applicable). Associate’s Degree required in the field of mental health, psychology, or a related field (behavioral health or human services) or a combination of education and experience that qualifies the candidate to receive an MHRT-C certification.
  • Certification: MHRT-C certification or (level A or B with a strategy to receive full certification)
  • Must have experience doing direct service with oppressed populations. Prefer case management experience with adults who have serious and persistent mental illness and have a history of trauma, substance abuse and misuse, homelessness, poverty, chronic pain and serious health issues. Deep understanding of recovery, trauma informed care, motivational interviewing, ACT and the strengths perspective and ability to build competency in these areas. Clear understanding and ability to navigate the mental health and health systems. Understanding of integrated health care and how to improve clients’ health outcomes.
  • Two years of experience required, three preferred.
  • Driving is an essential and regular part of your daily position responsibilities such are transportation of client for example case managers or driving to agency location to complete work for examples staff in facilities. A good driving record is required.
  • All candidates must be able to able to successfully pass an extensive criminal background check, child protective services and sex offender check and must not be on the state or federal suspension and disbarment list.

This is a 40 hour/week, full-time, hourly position located in Portland, ME.

The pay range is competitive and based on experience and education.
