  • The RN Case Manager administers nursing care to patients and/or plans, directs and coordinates the nursing care of a group of patients via the nursing process and in collaboration with the health team. The nurse also instructs patients and families in procedures and healthcare practices and makes referrals for continuing care after discharge. The nurse has frequent contact with patients, physicians, families and visitors and provides them with appropriate information, direction and any other assistance required. The nurse is expected to conform to the Hospitality Guidelines (located in the staff member handbook) and exhibit excellent guest relations. The nurse is expected to be flexible with time scheduling. The RN Case Manager will be aware of Major Hospital’s policies and procedures and assist in providing evidence based practices.
    MINIMUM QUALIFICATIONS
    • Professional & Technical Skills
      This position requires 1 year experience in a clinical setting such as Med/Surg, ICU, OB, etc. and/or Home Health Care.
    • Education
      Graduate of an accredited School of Nursing
    • License(s) or Certification(s)
      Must possess a current Indiana license as a Registered Nurse.
    • Other Skills or Requirements
      May be asked to use personal automobile for MHP business (MHP sites) and will be eligible for mileage reimbursement during the normal course of duties if safety criteria is met. Must have valid driver's license, current insurance, and acceptable driving record. Must adhere to safety guidelines as outlined in SPP: HR-44.
