SCOPE: Serves as an expert oncology population specific clinician, patient/client advocate and assists cancer/oncology patients/families to navigate the complex health care system in an organized, effective and efficient manner. This position will assist with the standardization and efficiency of care for patients in the cancer service line throughout the continuum of patient care. The Oncology Social Work Navigator models patient-centered care and assumes leadership responsibilities with high level contributions in direct patient interaction, the department, the service line, and across the institution.

FUNCTIONS:

  • Focuses on patient/customer safety
  • Provided clinical teachings to patients & caregivers
  • Delivers personalized service using MBP OLOL Service Standards
  • Provides for exceptional patient/customer experiences
  • Advocates for patients and their families in overcoming healthcare system constraints by facilitating needs, education, resource provision and/or representation within the multi-disciplinary care environment

MINIMUM QUALIFICATIONS

  • Associate’s degree in nursing
  • 1-3 years overall nursing experience
  • 1 year oncology nursing experience
  • Active RN license in Louisiana

PREFERRED QUALIFICATIONS:

  • Bachelor’s degree in nursing (BSN)
  • Experience as a nurse navigator, and/or patient educator in oncology
  • 2-5 years oncology nursing experience
  • Oncology Certified Nurse (OCN) or Navigation Certification

  • Demonstrates the skills and competencies necessary to safely perform the assigned job, determined through on-going skills, competency assessments, and performance evaluations..
  • Ability to effectively communicate with patients, physicians, family members, caregivers, and co-workers in a manner consistent with a customer service focus and application of positive language principles.
  • Demonstrates an effective community resource knowledge base and the judgment/ability to effectively select and
coordinate available resources, including referrals to regulatory agencies.
  • Possesses knowledge of health care financial and payer issues, and eligibility for state, local and federal programs.
  • Displays professionalism, remains composed under stress, and takes responsibility for delivering on personal and organizational commitments. Able to work independently; possess organization/time management and prioritization skills.
