Overview

St. Luke’s has an exciting opportunity for a Care Coordinator - Childrens, located in Mountain Home, ID! The Care Coordinator works with providers, care team, patients and families to implement a plan of care for assigned patients.

Job Functions

  • Works as consultant, educator, or community liaison to facilitate collaborative of patient care.
  • Evaluates patient progress toward outcomes by monitoring the goals, care plan and revising plan of care in consultation with clinical care team as necessary.
  • Assists the care team by identifying potential barriers that impedes access to care. Implements plan to address barriers.
  • Coordination of care with primary care and/or sub-specialty providers.

Required Criteria

  • Bachelor’s degree in Social Work or related field
  • 2 years’ of related experience.
  • American Heart Association (AHA) BLS Provider certification.
