Job Description

Health Educator-Chronic Disease Management

Health Educators educate patients enrolled in Alliance Plus Program on Chronic Disease Self-Management. Chronic Disease is further defined as diabetes, chronic obstructive pulmonary disorder, and congestive heart failure. Whether it be through warmth, compassion, attitude, or enthusiasm, you contribute in some way to make the patient experience better. As a Health Coach you will be meeting patient needs in a professional way.

Health Educators perform these essential duties as defined below:

  • Provide disease self-management education to patients face-to-face and telephonically (individual and group sessions)
  • Document interactions including patient action plan and tracker
  • Follow-up with patients via telephone including reminders/no shows, and responding to messages
  • Collaborate with Care Team in care huddles to provide insight on patient progress
  • Attend disease committee meetings to improve current processes
  • Collects physical assessment data
  • Participate in continuing education on disease self-management.
  • Identifies the targeted high-risk population within the practice site based on PCP referral, risk stratification, and patient lists.
  • Other duties as assigned

Other Responsibilities

  • Follows facility and OSHA safety rules and procedures while on assignment
  • Respects cultural and religious practices of patients
  • Upholds HIPAA regulations
  • Punctual and dependent for assigned/confirmed shifts

These are the qualifications that are necessary for someone to be considered for the position.

Qualifications include:

  • Experience with direct patient interactions.
  • 1 -3 years of experience.
  • Strong communication skills.
  • Bachelor's Degree-Health Administration, Health Promotion, Health & Wellness Required
