Bluestem PACE is seeking to grow our team with strong, team-oriented individuals. PACE is an innovative program designed to provide and integrate all acute, primary, preventive and supportive health services to high-risk individuals living in the community. Our goal is to keep our members living safely in their homes for as long as possible. We provide services in the community and at our PACE center which is open 8 AM to 5 PM Monday – Friday.

The Clinical Services Director oversees the day-to-day clinical operations of our program. This position is responsible for developing systems and procedures for maintaining the highest level of quality, customer service, teamwork, innovation, compassion, and accountability within all clinical services. They will assist leadership in policy formation, strategic planning as well as in the development and implementation of innovative programming to improve clinical practice. The Clinical Services Director will work closely with employees and program leaders to monitor, review and improve resource utilization.

The Clinical Services Director maintains a close partnership with our members, their families and the PACE Team. This position requires strong critical thinking and problem solving skills with a focus on preventing unnecessary emergency, hospital and skilled nursing utilization. The Clinical Services Director is an integral part of our interdisciplinary team. Within this team, the Clinical Services Director will participate through open communication, active listening, collaboration and multi-disciplinary planning. Successful candidates will exhibit strong clinical, management and leadership skills.

  • Licensed Masters Social Work;
  • Recreational Therapist;
  • Dietitian & Diabetes Educator;
  • Transportation Director/Drivers; and
  • Personal Care Aides
Qualifications

Education/Training/ Certifications:

  • RN degree from an accredited school of nursing or college/university
  • Strong critical thinking and problem solving skills desired

Experience:

  • 2+ years of nursing leadership/management experience preferred
