The MDS Coordinator position is responsible for the day to day activities of the MDS Program. This position will report to the MDS Program Manager and Pharmacy Site Manager. This position is responsible for building and maintaining positive relationships with the Funding Source, Referral Source, Community Mental Health Staff, Community Partners and Pharmacy Staff. The MDS Coordinator is also responsible for ensuring that the referred individual meets program eligibility/admission criteria, maintaining the program log, completing progress notes, participating on the MDS Treatment Team, conducting the Program Orientation Session, care coordination, collecting and entering data to be used for program evaluation/outcome reports.

The schedule is Monday - Friday 8:30-5:00 PM, no evenings, weekends or Major Holidays. Eligible for full benefits, PTO, 401k and more.

Major Duties & Responsibilities:

  • Review referrals for eligibility and program acceptance
  • Conduct the Program Orientation Session at the client’s home or other community locations
  • Maintain an accurate and current MDS Program Client Log on Spreadsheet or Database
  • Ensure client medical records are complete and accurate
  • Ensure ALL client documentation is completed and submitted in the required time frames
  • Monitor that the clients’ medications are delivered at the dosage and frequency indicated by the Treating Prescriber.
  • Participate in the client’s treatment planning sessions as a member of the MDS Treatment Team
  • Ensure the client information shared at the treatment planning sessions is accurate and current based upon conversations with the MDS Provider/Staff
  • Ensure Prescriber/MDS Treatment Team’s recommendations are shared with the MDS Provider/Staff.
  • Coordinate Care with client’s assigned case manager/support coordinator/support person through two way communication and follow up on client concerns/issues
  • Complete Discharge Note and disseminate to Treating Prescriber and Case Manager.
  • Gather evaluation and outcome data
  • Attend all Funding Source and Genoa Healthcare required trainings within stated time frames
  • Complete Monthly Review Forms based upon input from MDS Staff. The input may be communicated face to face, over the phone, or in writing.
  • Work collaborative with MDS Provider in ensuring clients receive Med Drops per Genoa Healthcare’s MDS Program Policy and Procedures.
  • Perform other duties as assigned

Educational or Skills Requirements:

  • Must meet the standards as a “Qualified Mental Health Professional” in Michigan’s Community Mental Health System.- See below
  • At least 1 year of experience as a case worker, preferably in the CMH System in Michigan
  • Experience working directly with adults with a serious mental illness
  • Excellent organizational and time management skills
  • Ability to work independently and with a team
  • Ability to communicate clearly, concisely and professionally with health care providers
  • Highly proficient in Microsoft Products including Word and Excel.
  • Ability to type and complete accurate data entry
  • Willingness to provide home/community visits

Qualified Mental Health Professional Standards:

Qualified Mental Health Professional (QMHP) - Individual with specialized training or one year experience in treating or working with a person who has mental illness; and is:

  • a psychologist
  • a physician,
  • an educator with a degree in education from an accredited program,
  • a licensed or limited licensed social worker,
  • a physical therapist,
  • an occupational therapist,
  • a speech-language pathologist
  • an audiologist,
  • a registered nurse,
  • a therapeutic recreation specialist,
  • a licensed or limited-licensed professional counselor
  • licensed or limited licensed marriage and family therapist, OR
  • a licensed physician’s assistant

OR An individual with a bachelor’s degree in a human services field who was hired prior to January 1, 2008 and performed in the role of a QMHP prior to January 1, 2008 would also qualify. NOTE: If an individual was hired and performed the role of a QMHP prior to January 1, 2008 and later transfers to a new agency, his/her QMHP status will be grandfathered in to the new agency.

OR Social Worker - Individual who possesses Michigan full or limited licensure as a master’s social worker or a bachelor’s social worker. Social workers with limited licenses must be supervised by a fully-licensed master's social worker.

OR Student Intern**** - Individual who:

  • is a student in one of the following health profession training programs: counseling; marriage and family therapy; psychology; or social work which has been approved by the appropriate board, and
  • is performing the duties assigned in the course of training, and
  • is appropriately supervised according to the standards set by the appropriate board and the training program.

****Social work student interns must be pursuing a master’s degree in social work and be supervised by a Licensed Master’s Social Worker in a manner that meets the requirements of a Council on Social Work Education (CSWE) accredited education program curriculum that prepares an individual for licensure.

****ONLY Social Work Student interns pursuing a Master’s degree in Social Work qualify for the MDS Coordinator position
