Social Worker Job Opening

Seasons Hospice & Palliative Care is seeking a Social Worker to join our team of professionals. We make a difference every day to patients and families coping with end of life illness in creating a center of excellence in hospice care.

The Social Worker supports the business development activities of the organization. As a member of the hospice interdisciplinary team, provide social service counseling and guidance to facilitate the maximum function and coping capacity of each patient and family member while following all hospice policies and procedures, in accordance with the interdisciplinary plan of care. Support the Provider Relations activities of the organization. Provide bereavement counseling to family members as identified.

Must have a minimum of a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education.

Qualifications for state-specific requirements above Seasons minimum education standards are as follows:

o AZ: Must be licensed by the state at a minimum of LMSW.
o CT: Must have a minimum of a Master of Social Work (MSW) and be licensed as an LCSW in the State of Connecticut; or have an MSW and be directly supervised by an LCSW on staff.
o Minimum of three years’ experience in a health care setting.
o IL: Must be licensed in the state as an LSW or LCSW.
o MA: Must be licensed to practice social work in the state as a Licensed Independent Practitioner of Clinical Social Work, or be a Licensed Certified Social Worker.
o MI: Must be licensed in the state as an LSW, LMSW, or LCSW.
o MD: Must have clinical experience in counseling and casework and be licensed by the state as an LGSW, LCSW, or LCSW-C.
o NJ: Must have an approved license by the NJ Board of Social Work Examiners.
o OR: Must have CSWA.
o TX: Must be licensed to practice social work in the state.
o WI: Must be certified/licensed in the state

Minimum one year of social work experience in a healthcare delivery system is required; two years’ experience is preferred.

Must have reliable transportation, a valid driver’s license, and the minimum state required liability auto insurance.

Additional Qualifications for a Level II Social Worker:

Must hold the highest level of licensure within the state, as indicated in the table below; and/or currently hold the ACHP-SW advanced certification.

Must have greater than two years of full-time experience as a social worker.

Additional Qualifications for a Level III Social Worker:

Must hold the highest level of licensure within the state, as indicated in the table below.

Must hold the ACHP-SW advanced certification.

Must have greater than five years of full-time experience as a social worker
