Community Health Worker
Kalihi Palama Health Center
• Honolulu, HawaiiPrimary Purpose:
Provide case management, outreach, and health education and interpretation services. Assist in training students in the health professions and health care professionals regarding cultural issues and health care. Serve as an advocate for appropriate, quality health and social services. Each team member plays an integral role in providing patient-centered health care.
Essential Duties:
- Interpretation services in specified language needed
- Provide oral and written interpretation services, providing health education and counseling as needed.
- Assist in scheduling patient for appointments
- Assist patient and connect to appropriate staff when requesting for medication refills
- Assist patients in application for entitlements: medical insurance, financial, housing etc.
- Provide outreach and home visits to patients who are non-complaint with treatment or when deemed necessary by health care professionals
- Provide health education and counseling to patients
- Assist patients in filing out intake forms and other pertinent forms needed for the office visit
- Assist patients with outside billing problems
- Document all pertinent requests and contacts in patients chart
- Provide referral and linkage when deemed appropriate by health care team
- Make follow-up call or write reminder letters to patients
- Assist in training students in health professions and health care professionals as well as patients regarding cultural issues and health care
- Attend trainings and classes for the improvement of their services
- Create a safe and clean environment
Regular and reliable on-site attendance
Other Duties:
Perform other related duties as assigned
- Observe safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly.
- Our model uses the parallel workflow design where workload is distributed throughout the team, thus improving efficiency resulting in better workflow and improved patient and staff satisfaction.
Minimum Qualification Requirements:
Education:
High School Diploma or GED required.
Certification as a professional translator preferred but not required
Certification as a professional community health worker preferred but not required
Post-secondary education preferred.
Experience:
One year of experience in providing bicultural and bilingual services in a similar setting.
Formal training in medical terminology preferred but not required
Formal training as a community health worker preferred
Skills/Knowledge:
Knowledge of the community and community service resources especially in Kalihi-Palama area.
Fluency in English and either Cantonese or Mandarin required
Fluency in Vietnamese is required.
Ability to read, understand and translate technical/medical instructions furnished in written, oral or diagram form.
Basic knowledge of computer skills preferred but not required
Hawaii Driver's License preferred but not required
